Here, selected responders at field offices download enrollment forms linked to the company's intranet with instructions on how to complete the form.

After completing the form, a responder e-mails the form back to the company. A validity check is performed prior to transmission to make sure that all required field entries have been completed in the proper format. The data is then parsed and stored in a database. An e-mail advisory is sent back to responders to confirm or deny the application and convey any follow up requirements. Advanced reports as described in the Access database section of our web site can be created as desired.
